
Boxing medicals

South Bristol Sports Clinic is pleased to support local Amateur Boxing Association Clubs, and our resident doctor, Heather Montague, has eight years’ experience carrying out medicals and providing cover for local competitions. Medicals take about 20 minutes to complete.
You need to arrive with:
Your ME1 form, with the first two pages already completed
Birth certificate, passport or photo driving licence-
Urine sample – or be able to provide one on arrival (sample pots available at reception)-
Cash or cheque made payable to TeamWorks (GP) Ltd
Please wear or bring along the shoes you spend most time wearing (e.g. work or school shoes rather than sports shoes). Please bring a parent, guardian or carer if you are under 16.

Director of TeamWorks (GP) Ltd which provides medical support for athletes, performers and therapists.
Dr Montague currently works as a part-time GP at the Knowle NHS Walk-In Centre and works for the Football Association as the lead England Disability Football Team Doctor and as the England Futsal Team Doctor. She has worked asa sports doctor since 1993, was the England Cadet and Junior Karate Team Doctor 2001-2004 and the Bristol City Football Club Doctor 2001-2008.
